A-Line Staffing is now hiring  Clinical Incident Investigator (LPN/LSW) in Ohio . The  Clinical Incident Investigator (LPN/LSW)would be working for a Fortune 500 company and has career growth potential. This would be full time / 40+ hours per week.

**Must have knowledge with Ohio MyCare Waiver and Ohio’s Incident reporting process

Clinical Incident Investigator Compensation

• The pay for this position is  up to $38.00-$42.00 per hour Based on Experience
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Clinical Incident Investigator Highlights

• Schedule: The required availability for this position is  Monday–Friday, 8:00 AM – 5:00 PM EST
• Location: Must reside in and have Ohio license and knowledge of Ohio MyCare Waiver) – 100% Work from Home

Clinical Incident Investigator Responsibilities

• Conduct waiver incident reporting, investigation, processing, and closure activities
• Review claims data, clinical records, case management notes, and assessments
• Determine if incidents meet criteria for critical incident classification
• Complete investigations within required timelines (45 days)
• Collaborate with care coordination teams, providers, and external partners
• Develop and ensure implementation of person-centered prevention plans
• Ensure compliance with Ohio Administrative Code, CMS regulations, and provider agreements
• Submit incidents into the Ohio Department of Medicaid Incident Management System (IMS)
• Maintain accurate documentation and meet performance metrics for timelines and quality
• Communicate with members, providers, and partners as needed via phone, email, or fax

Clinical Incident Investigator Requirements

• Active Ohio licensure required:   LPN, LSW, LISW, LPC, or LPCC (Ohio)
• Minimum 2+ years of experience in field of licensure with clinical experience
• Strong knowledge of  Ohio MyCare, Medicaid products, and incident reporting processes
• Strong critical thinking skills and ability to work independently
• Proficiency with Microsoft Office (Excel, Word, Outlook, Teams)
• Tech-savvy with ability to navigate multiple systems (Trucare, IMS, etc.)
• Attendance is mandatory for the first 90 days

Clinical Incident Investigator (RN/LSW) Preferred Qualifications

• Managed care experience preferred
• Ability to work independently while contributing to a collaborative team environment
• Strong accountability, communication, and organizational skills
• Experience working in remote environments
