YNHS Position : Psychiatric Nurse Practitioner (PNP)

Examples of Duties (this list may not include all duties assigned):
Provide diagnostic consultation to primary care provider team.
Provide therapeutic interventions including brief counseling and crisis management.
Collaborate with integrated care teams to address behavioral health needs of patients.
Screen patients for behavioral health conditions and refer to appropriate services.
Coordinate care with internal providers and external mental health/social service agencies.
Participate in multidisciplinary case reviews and care planning.
Maintain accurate and timely documentation in the Electronic Medical Record (EMR).
Ensure patient confidentiality and compliance with HIPAA regulations.
Participate in quality assurance activities and peer review.
Represent YNHS in community meetings and collaborative networks as requested.
Support education and training initiatives for staff and patients related to behavioral health.
Provide services at both clinic and satellite locations, including outreach to homeless populations.
Assist patients, and families as appropriate, to interpret and manage maladaptive behaviors and to plan healthy lifestyle changes.
Work with patients to improve adherence to medical recommendations and medication compliance.
Conduct psychiatric evaluations and develop individualized treatment plans.
Prescribe and manage psychotropic medications in accordance with Washington State law and clinical guidelines.
Collaborate with primary care medical providers and mental health staff in the design and delivery of patient care.
Participate in the agency’s 340B Pharmaceutical program for best outcomes to the patient and the program.
