Job Description

Centers Health Care is seeking a dedicated Registered Dietitian to join our Healthcare & Medical Services team and support patients across a range of clinical settings. In this role, you will apply evidence-based nutrition principles to assess patients’ nutritional needs, develop individualized care plans, and collaborate with an interdisciplinary care team to promote optimal health outcomes. You will conduct comprehensive nutrition assessments, interpret clinical and laboratory data, and provide medical nutrition therapy for patients with acute and chronic conditions such as diabetes, cardiovascular disease, renal disease, gastrointestinal disorders, malnutrition, and obesity. As a Registered Dietitian with Centers Health Care, you will provide one-on-one counseling and group education, translating complex nutrition science into practical, culturally sensitive guidance that patients and families can understand and use. You will document all assessments, interventions, and patient progress using electronic health record systems, ensuring accurate, timely, and compliant records that meet regulatory and accreditation standards. You will also play an active role in interdisciplinary rounds, care plan meetings, and case conferences, offering expert nutrition input to physicians, nurses, therapists, and other healthcare professionals. You will participate in menu planning and review to help ensure meals meet clinical, regulatory, and safety standards while also supporting patient preferences and quality of life. You may assist in monitoring food service operations, conducting nutrient analyses, and supporting adherence to infection control, sanitation, and food safety policies. In addition, you will contribute to quality improvement initiatives, help develop and update nutrition-related policies and protocols, and identify opportunities to improve patient satisfaction and clinical outcomes through nutrition interventions.
